Module Special Admissions Requirements
Must be on a Foundation degree leading to assistant practitioner status
Module Additional Assessment Details
2000 word written assignment (weighted 100%) ( addressing all LOs)
The assignment should identify a long term condition pertinent to the area of work and how the Assistant Practitioner role contributes to service education and maintenance of independence of a client with that condition. A definition of a long term condition and a review of authoritative literature relating to incidence and current practices will be used to underpin the care and advice consummate to the level of an Assistant Practitioner.
Formative assessment will be an informal group discussion regarding a case study of a patient with a long term condition whom they have cared for, feedback will be given via group and individual discussions, group tutorials and class based activities
Module Indicative Content
Include all relevant content as referred to in the learning outcomes and assessment.
This module will provide an overview of the patho-physiology and impact of the most common long term conditions identified either through the Quality and Outcomes Framework or other government initiatives. It will enable trainee assistant practitioners to demonstrate underpinning knowledge and skills to support patients and registered professional staff in their own work settings. This module will enhance clinical skills in monitoring long term conditions. There will be a strong emphasis on disease prevention and the promotion of independence and self care within these patient groups.

- The Cardiovascular System
- Central Nervous System
- Respiratory System
- Diabetes
- Obesity
Other long term conditions will be explored appropriate to the work based of the student

Academic: Professional development and awareness through 4 taught days within the university to support the student's placement experience. There is an assessed presentation at the end of the placement based on a Critical Incident and a written submission to support the presentation.
