Module Special Admissions Requirements
Must be enrolled on FdSc Integrated Care and in employment in a health care setting with access to a mentor
Module Additional Assessment Details
A 10 minute oral presentation weighted at 50% addressing LO's 1, 2 and 3

A 1,500 word reflective essay weighted at 50% addressing LO 4

Additional Assessment Details (include formative feedback / assessment):
Formative feedback will be given via classroom discussion, peer and self evaluation, individual and group tutorials. Students will undertake one 10 minute formative presentation. Formative feedback will be given on reflective essay plans
Module Indicative Content
This module will enable students to explore the role of the Assistant Practitioner, encompassing accountability and professional responsibilities and boundaries, the values of integrated care and contemporary health and social care policies which promote integrated care. Students will explore the values that underpin holistic care, the multidisciplinary and multiagency approach required to promote individualised care and will reflect upon care pathways that support the service user and their carer(s) during their healthcare journey
