LEARNING OUTCOMES
1. Analyse the complexity of working within the community settling and identify the skills knowledge required by the nurse to deliver and manage patient care. (Analysis, Knowledge and Understanding)

2. Discuss effective communication with multidisciplinary and multi-agency teams in order to recognise the importance of inter-professional working within public health strategies. (Communication)

3. Critically evaluate the appropriateness and application of established health promotion approaches, including an analysis of the concept of `self care', and their effectiveness in community health settings.

ADDITIONAL ASSESSMENT DETAILS
An Assignment length 3000 words weighted at 100% assessing all LO's

This assignment requires the student to explore the unique role of the nurse working within a community base care setting. They are required to consider the current context of community based care including emerging issues and the shared professional challenges that these may present to nurses working within the community based services followed by, a more detailed exploration of an aspect of community nursing relevant to the student's chosen field (adult, child, mental health).
Consideration must be given to key professional issues and strategies which may be employed by the community nurse to overcome these.

Formative Assessment:
feedback will be incorporated into the seminar and tutorial sessions via group discussions and workshops
INDICATIVE CONTENT
The aim of this module is to develop the understanding of public health and understanding of care in the community first considered in year one. This module will enable to student to develop nursing interventions in a community environments across the lifespan.

- Public health principles, priorities and practice and their relationship to the major causes and social determinants of health, illness and health inequalities.
- Continuing health promotion and nursing care to support people with complex health needs, vulnerable groups, communities and populations, whose lives are affected by ill health, disability, inability to engage, ageing or death
- Inter-professional and partnership collaboration, to support service users, carers, groups, communities and organisations to manage risk and ill health, and empower choices that promote safety and self-care where possible
- Nursing roles, responsibilities and functions in a community care setting, and adaption of practice to meet the changing needs of people, groups, communities and populations, role modelling and effective communication
- Contemporary evidence based practice in the community, required to plan, deliver and evaluate person-centred care, communicate findings, influence change and promote health and best practice, in partnership with others involved in the care process, to ensure high quality care. Clinical decision making complexity, requiring specialist knowledge and expertise, and consultation and referral processes
- Comprehensive, systematic nursing assessments that take account of relevant physical, social, cultural, psychological, spiritual, genetic and environmental factors, in partnership with service users and others through interaction, observation and measurement
- Educational support, facilitation skills and therapeutic nursing interventions to optimise health and wellbeing, which promote self-care and management whenever possible, helping people to make choices about their healthcare needs, involving families and carers where appropriate, to maximise their ability to care for themselves
- Safeguarding issues in the community

There has been an ongoing ambition to provide more care in the community and closer to patient’s homes. As such a significant portion of health and social care is provided in the community by a wide range of services.

The community based care module is a core module taught within the second year of the pre-registration nursing programme and its content is relevant to the adult, child and mental health fields of nursing.

The module will introduce you to a wide range of nursing roles providing care in the community and will enable you to explore key principles which are important to consider when working with individuals, families and communities. This includes the requirement for community nurses to be involved with early intervention and preventative health care, as well as collaborating with a wide range of services to provide patient centred care.

You will explore a range of key emerging issues which are impacting on services in the community and as well as some of the challenges and professional issues which may be faced by nurses when providing community based care.

Teaching on the module is organised to help you work towards meeting the intended learning outcomes. Successful completion of this module is an essential element of the BSc (Hons) Nursing programme for all adult, child health and mental health fields.
