Portfolio of evidence should include:
Testimonies of clinical practice, reflective diaries, reflective writing, personal development plans, completed evidence grids relating to each of the module learning outcomes, SWOT analyses and action plans linked to SWOT’s
Formative Assessment:
A group presentation of 10-15 minutes' duration based on student's experiences of team working during the module and related to the module learning outcomes. The presentation will be peer and self-assessed

INDICATIVE CONTENT
The aim of this module is to enable students to integrate their learning and experience of nursing older people, developing their ability to manage, analyse and evaluate person centred nursing care within the context of the wider health and social care agenda. In order to achieve this the indicative content is as follows:
- Physiological, psychological, sociological, spiritual and cultural aspects of ageing to include Ageing and Mental health/ Ageing and physical health / Altered presentation of symptoms (across acute and care closer to home)
- Demographic changes and health inequalities linked to ageing
- Neurological conditions including stroke / Long Term conditions
- The experience of Ageing with a disability / developing disability later in life/ Ageing in context/ Ageing and Learning disability
- Dementia/ Depression/ Delirium / BPSD /Malignant social psychology
- Falls / ADL's i.e. nutrition/ continence / mobility/ sexuality etc
- Reciprocity and the use of biography in enhancing and promoting quality of life
- Relationship / person centred and family centred care
- Communication /Care, compassion and dignity
- Organisational culture and creating positive cultures in health and social care settings (ie senses framework ) including environments of care/ Transitions in care settings
- Policy and Guidance and their impact on caring for older people in health and social care
- Psychological resilience
- Research led teaching and learning - i.e. centre for ageing etc. i.e. RAID and similar projects
- Transgenerational care, caring for the carers
- Loss, grief and bereavement
- Ethical dilemmas

LEARNING OUTCOMES
1. Demonstrate knowledge and a critical understanding of how to promote the rights, choices and wishes of older people, paying particular attention to equality, diversity, dignity, respect and relationship and person centred care alongside the needs of an ageing population (Knowledge and Understanding)
2. Critically evaluate the effectiveness of a range of strategies to promote rights, choices and wishes of older people in a relation centred way (Analysis, Application)
3. Demonstrate the knowledge, critical understanding to systematically carry out accurate holistic assessments of older people using appropriate diagnostic and decision-making skills, providing effective care for service users and carers who may have complex care needs (this includes both mental and physical health) (Application, Enquiry, Knowledge and Understanding)
4. Reflect on and analyse the need for effective leadership in managing the nursing care of the older person, understand and coordinate inter professional care when needed, and liaise with specialist teams. (Analysis, Reflection)

Demonstrate the ability to listen with empathy and respond positively to older people who may be anxious, distressed, or facing problems with their health and wellbeing understanding the need for care, compassion and effective communication with service users, carers and other health and social care providers.
