Practice assessment: - this is done through the MYE PAD DOCUMENT which is a record of achievement of the Professional Values in practice and NMC 2018 Future Nurse: Standards of Proficiency Part 2 (LO 1 and 4).

Written Reflection: completion of two pieces of reflection (one for Practice Learning Period 4 and one for Practice Learning Period 5 each being 1000 words). These will cover two platforms within the NMC 2018 Future Nurse Standards of Proficiency: Platform 3- Assessing needs and planning care and Platform 6 - Improving safety and quality of care (LO 2)

Episode of care - The practice assessor/supervisors and student will identify an appropriate episode of direct care involving caring for people with increasingly complex health and social care needs. Professionalism underpins all aspects of the student’s performance. The aim of this assessment is to demonstrate the student’s progression within the NMC 2018 Future Nurse: Standards of Proficiency (LO 3).
