Assessment 1

Objective Structured Clinical Examination (OSCE)

Skills Proficiency Test

Assesses module learning outcome 1.

You will complete three OSCE stations designed to evaluate your ability to perform complex clinical skills across a range of acute and minor presentations. Each OSCE station will last approximately 10 minutes and require demonstration of sophisticated technical skill, clinical reasoning, differential decision-making and safe adaptation of interventions in response to changing patient needs. Examiners will assess your ability to justify actions through reference to evidence, guidelines and professional standards.

Each station is 10 minutes in duration.

You will demonstrate the allocated skill to a trained assessor in a simulated clinical environment.

Performance is measured against a binary competency checklist (Pass/Fail), assessing accuracy, safety, infection prevention, equipment handling, and adherence to professional standards.

Rationale:

At Level 6, you must demonstrate mastery and critical evaluation of clinical skills rather than simple performance. OSCEs allow for consistent, high-fidelity assessment of advancing practice capability and preparedness for the transition to autonomous practice.

This assessment requires you to manage a complex simulated patient encounter that reflects the realities of advancing paramedic practice. You must perform a comprehensive assessment, synthesise clinical findings, justify decisions using legal, ethical and professional frameworks, and initiate autonomous therapeutic interventions supported by evidence and national guidelines. You will be expected to demonstrate leadership in managing the clinical environment and articulate the rationale underpinning all decisions during examiner questioning.

This simulation assesses the integration of high-level assessment skills, ethical and legal awareness, and autonomous clinical decision-making—key capabilities for Level 6 paramedic practice. It provides a safe, controlled environment in which you can demonstrate readiness for professional registration and independent practice.

This module develops advancing clinical, analytical and autonomous decision-making skills required for contemporary paramedic practice. Content includes:

Advancing Clinical Assessment and Management

Comprehensive assessment techniques for complex, multi-system and atypical presentations.

Differential diagnosis, red-flag recognition and escalation decisions in high-acuity, low-frequency events.

Evidence-informed therapeutic interventions across acute, urgent and minor presentations.

Analysis of clinical guidelines, research evidence and risk-benefit considerations in autonomous care.

Professional, Legal and Ethical Decision-Making

Application of HCPC standards, case law, professional accountability and consent principles in autonomous interventions.

Ethical reasoning in complex and uncertain scenarios, including capacity, safeguarding, and balancing risk.

Clinical Reasoning and Synthesis of Findings

Integration of physiological data, patient history, exam findings and digital decision-support tools (e.g., JRCALC) to form defensible clinical decisions.

Advancing formulation of care plans, therapeutic pathways and risk-stratified management options.

Human Factors, Leadership and Situational Management

Leading clinical episodes, coordinating multidisciplinary involvement, and managing dynamic, high-pressure environments.

This module challenges you to assess and manage complex and atypical patient presentations, justify therapeutic interventions, and apply professional, legal and ethical frameworks to high-stakes decisions. Through immersive advancing simulation and evidence-informed practice, you will integrate clinical findings, lead dynamic scenarios and demonstrate the confidence and competence expected of a graduate paramedic preparing for registration and independent practice.
LEARNING STRATEGIES
Learning in this module focuses on developing the advancing clinical reasoning, assessment capability and autonomous decision-making required for professional paramedic practice. Teaching will be delivered through high-fidelity simulation, advancing skills workshops, case-based learning and focused lectures that introduce complex clinical, legal and ethical concepts relevant to independent therapeutic intervention.

Simulation will form a central component of learning, providing opportunities to manage complex and atypical presentations, synthesise clinical findings and justify decisions under realistic levels of pressure. Structured debriefing will support critical reflection, error recognition and development of professional judgement.

Interactive seminars and case analyses will encourage you to evaluate contemporary evidence, challenge assumptions and apply human factors principles to improve performance in dynamic clinical environments. Digital resources and pre-session materials within the VLE will support self-directed learning and enable you to prepare for advancing clinical scenarios.

As you move towards transition to registration, you will be encouraged to take increasing ownership of your learning, engage with feedback at an advancing level and demonstrate leadership behaviours, situational awareness and accountability consistent with autonomous professional practice.
