Module Additional Assessment Details
A group assessment and care planning project which is an indepth exploration of the issues and experience of the community care process (assessment, planning and provision)

Each case study will consist of:
a. A video of a simulated community care assessment visit based upon analysis of case materials.
b. A set of completed community care assessment documentation. 1500 words.
c. A written rationale for the proposed assessment and care plan. 1000 words.
d. A group presentation to the assessment panel outlining the community care assessment process undertaken. (20 mins) identifying how team working skills were used in order to problem solve jointly in the case scenario.

Students must pass all elements of the assessment in order to pass the module.

Module Special Admissions Requirements
This module is open only to students enrolled on the BA (Hons) Social Work.
Module Indicative Content
The historical and policy context of community care;

Models and theories of ageing, disability, mental health and multiple oppression, and their implications for practice;

Working with diversity in community care:-personal, cultural, religious and spiritual

The evidence base for work in community care, including:
- The impact of social factors on patterns of need and vulnerability;
- The impact of professional interventions;
- The concerns of service users;

Care management, including assessment and care planning;

Issues for carers, and carers assessments;

Multi-disciplinary working;

Rights, civil liberties and capacity for consent and decision-making;

Strategies for involving and empowering service users, including:
- Advocacy and self-advocacy;
- Involving service users in training and service planning;
- Supporting service users to manage Direct Payments;
- Supported decision-making.

Understanding and working with risk.

The abuse of vulnerable adults, and the social work role in adult protection.

Module Learning Strategies
Contact Hours (70)
Whole group sessions - consisting of participatory lectures and presentations by visiting speakers in order to disseminate core knowledge and theoretical perspectives, including those arising from the experience of service users - 11 x 5hrs per week = 55 hours

Facilitated small group sessions - following a Problem Based Learning exercise on a case study - to include group exercises and discussion, reflection on practice in the light of knowledge and understanding acquired during the module, relating this to values and ethics - 5 x 2 hrs per week = 10hrs

Facilitated role play sessions taking place in (Forensic House Skills Lab) where small groups have an opportunity to practice interviewing and assessment skills. 5 hours

Some materials will be accessed through Blackboard / VLE.

Directed study and independent learning - preparation for the group task including participation in small group sessions through guided reading, library / Internet research, contact with outside organisations - 235 hours.
